HYPERBARIC OXYGEN THERAPY IN TRAUMATIC BRAIN INJURY

Author: Daniel KRAUS, Juraj ČELKO

Abstract: Background: Traumatic brain injury (TBI) is the leading cause for mortality and morbidity for millions of young people around the world every year. A decrease of oxygen supply in TBI has devastating effects on the injured brain tissue.

Objectives: This study is aimed to determine whether the hyperbaric oxygen therapy (HBOT) can be suggested as a scientific and effective treatment for TBI.

Methods: In the review article, we present findings from studies published in electronic databases and journals until the end of 2016 aimed at the effect of HBOT in TBI and we demonstrate two patients with TBI that were treated with HBOT in the hospital of Trenčín.

Results: Many articles have been published that demonstrate HBOT could affect brain tissue hypoxia thereby avoiding neuronal cell death through increased cerebral oxygen metabolism. Early and timely HBOT intervention can have a more robust effect than delayed intervention, but HBOT is also effective in repairing an injured brain even long after that injury took place. In two patients that we demonstrate in this article, daily application of HBOT was prescribed not only while in hospital but at certain intervals also later and it contributed significantly to restoration of neurological and cognitive functions.

ENDOPROSTHESIS OF RHEUMATIC HANDS AND FOLLOW REHABILITATION CARE

Author: Katarína KOVÁČOVÁ

Abstract: Hand injury is one of the worst impairments from a functional point of view. In our work we have chosen the issue of the hands of rheumatism. Hands of rheumatism after repeated inflammatory processes affect various deformities. These deformities have been caused caused to the destruction of ligaments and joints and the following instability of the small joints of the hands appears. This led orthopedists and surgeons to various arthroplasties, arthrodesis to total joint replacements. Then total metacarpophalangeal (MCP) endoprostheses (TEP) have become a possibility of rheumosurgical treatment. In our work, we obtained a sample of five men and women aged 40-70 years after implantation of TEP MCP joints. Their early treatment after surgery took place in the National Institute of Rheumatic Diseases in Piešťany. An indication of the operation was ulnar deviations with instability and degenerative changes in MCP joints. Post-operative care in such patients has its own specificities when we look at the overall condition of the patient and manage the current state. Pain and frequent inflammatory processes accompany rheumatic patients in everyday life. It reflects in the psychic and affects the overall condition of the patient and therefore rehabilitation co-operation. In spite of everything, the postoperative status is a priority and requires a physiotherapist's unique approach. Appropriate selection of procedures and patient collaboration will reduce pain, increase muscle strength, keep or improve the range of movement and thus improve self-care or self-sufficiency of the patient.

LUMBAR LAMINECTOMY AND AFTERCARE

Author: Jana KORCOVÁ, Patrícia SHTIN BAŇÁROVÁ, Iveta PETRÍKOVÁ ROSINOVÁ

Abstract: Most of the population suffers from a back pain during their lives. The initial problem is the lack of the physical activity among people and the lack of knowledge about how to protect our back or about the principals of back school. Patients with back problems in general still increases and the intervention of specialists and the physiotherapists will be needed more. If conservative way of treatment does not help, the surgery is indicated. One of these surgical procedures is the laminectomy. It is a surgical intervention where the lamina or the part of it (hemilaminectomy) is removed or trimmed to widen the spinal canal and create more space for the spinal nerves. The laminectomy indications differ. But difficult degenerative disc disease, disc herniation, spondylosis, or spinal stenosis. The great role after the laminectomy plays proper physiotherapy.

THE RETROSPECTIVE STUDY OF INTERVENTION METHODS IN PATIENTS AFTER SURGICAL THERAPY OF BREAST CANCER

Author: Jana SLOBODNÍKOVÁ

Abstract: Introduction: In the Slovak Republic, breast cancer is the most common type of oncological disease in women, the leading cause of cancer mortality in all women. In the past 10 years, breast cancer mortality has continued to slowly decrease, the breast cancer incidence in the past 20 years has continued increase, likely due to program of preventive mammography.

The Aim:  The aim of this retrospective study was the asses the importance, the position of the interventional methods – core cut biopsy and fine needle aspiration biopsy under ultrasound control in the differential diagnostics of changes after surgical treatment for breast cancer

Patients and Methods: We evaluated examinations of 385 patients with newly diagnosed breast carcinoma after conservative surgical therapy in period between January 2005 and December 2016 from Clinic of Radiology in Trenčín. In all cases was performed ultrasound, mammography 2D and 3D examinations, ultrasound guided biopsy with free hand methods with needle 14, 16 and 20 G.

Results: The major findings as postoperative changes were seromas, hematomas, fat necrosis, skin thickening, inflammatory changes, granulomas, postoperative scars and local tumor recurrence.

Conclusion: The ultrasound guided interventional methods, fine needle aspiration cytology and core cut biopsy are reliable and perfect methods for the differential diagnosis of postoperative changes and the follow up of postoperative changes.

THE ISSUE OF DIAGNOSTIC PROCEDURES AND TREATMENT INTERVENTIONS OVERUSE IN HEALTHCARE

Author: Ján BIELIK

Abstract: Background: Medicine and healthcare are characterized by increasingly more advanced technology that allow for more accurate diagnosis and better treatment. The issue is that this trend is faster than the increase of financial resources to healthcare systems.

Methods: Literature found by researching specific (re)sources were used to find out the current situation in Slovakia and worldwide.

Results: Formalized, RESP. institutionalized activities and initiatives for professional determination of the state of overuse, overdiagnosis and overtreatment already exist around the world. In Slovakia, individual interests in said issue is more commonly observed. Despite that, the studies about using laboratory examination of thyroid diseases assessed the estimate saving potential about 10-12% a year, which can mean approximately 900 000 € a year.

Conclusions: Slovakia should follow the worldwide trend and create a method of acquiring reliable results about the possibilities of saving by rational indication in diagnosis and treatment of chosen diseases and without harming the patient. The premise is the cooperation of Ministry of healthcare, health insurance companies and scientific medical companies.

THE BENEFITS OF STANDING IN THE SHADOW OF MY NEIGHBOUR: EXPERIENCE OF A CHRISTIAN COACH

Author: Eva KRÁLOVÁ

Abstract: The contribution focuses on Christian coaching which is emerging in Slovakia mainly as the form of service. In the introduction authoress gives the definitions of life coaching, Christian coaching, and briefly differentiates what makes life coaching Christian. Christopher McCluskey defines Christian coaching as “a way of helping people find God´s vision for their lives and learn to live accordingly.” The first chapter approaches the added value of Christian coaching dialogue which leads to four unique characteristics of a Christian coach: A) Brings biblical world-view to the coaching dialogue; B) Reflects the qualities of Jesus Christ; C) Christian coach is not fully neutral: Is happy when clients move toward balance with biblical teaching; D) Prays for the clients. In the second chapter the authoress explains an important part of coaching dialogue, powerful questions that help clients process and move forward. Colins, G. R. created the following four quadrants with broad open-ended questions Four Quadrants of Christian Coaching that can be successfully used during coaching dialogue: 1. Awareness: Where are you now? → 2. Vision: Where do you want to go? → 3. Strategy and action: How do you get there? → 4. Obstacles: What might get in the way? The contribution is concluded with the brief description of survey results. The aim of the survey is to examine what a Christian coach gains by his or her voluntary retreat in the shadow of the client. This “retreat” is important, because during coaching dialogue a coach cannot interfere with the thoughts of the clients and should respect their independence and decisions.
